How to Evidence AI Actions for Regulators and Auditors

Evidencing AI actions for regulators and auditors is not a reporting task. It is a recording task. If the evidence is not captured at the moment of the decision, it cannot be reconstructed later. AI governance must produce evidence by design, not by request.

Every authority decision must be recorded. The request (what the agent asked to do), the governance context (the agent's inventory record, risk classification, ownership and policy), the authority decision (allow, hold, transform or deny), the rule that drove the decision, and the outcome (what actually happened) must all be captured.

Evidence must be tamper-evident. If evidence can be edited after the fact, it is not evidence. ServiceNow provides the audit trail infrastructure. VORTIQ-X writes decisions to that trail. The combination produces evidence that auditors can rely on.

Evidence must be queryable. Auditors will ask specific questions: show me all actions by this agent in this period, show me all denied actions, show me all actions above this financial threshold. Evidence must be complete. A record that captures the decision but not the context is incomplete. An auditor needs to see not just what was decided, but why. The governance context (risk classification, ownership, authority boundaries) is part of the evidence.

Evidence must be exportable. Regulators may request evidence in a specific format. The ability to produce a regulatory audit pack from live records, rather than compiling it manually, is the difference between a governed AI programme and an ungoverned one.
